Athletic Forms

For the 2025-26 school year, the forms listed below are required for students to participate in Athletics.  This letter from Bunger Athletic Director Anthony Thomas outlines current changes to required athletic forms.

Athletic Form packets are available at the Athletic Office and can be returned to the Registrar’s Office when school starts in the fall.
